Step 1 of 5This procedure outlines the steps required to locate and access the Major Repair Expenditure form within the Refresco FIZZ portal.
This SOP applies to all employees who need to submit a Major Repair Request for equipment, assets, or infrastructure requiring significant maintenance or repair.

Step 2 of 5Open a web browser.
Navigate to: https://fizz.refresco.com
Once logged in, go to the My BU and then Digital Forms menu.
Browse the list of available forms.
Within Digital Forms, locate Major Repair Request.

Step 3 of 5Attachments must be submitted in PDF format only.

Step 4 of 5The workflow is governed by the approved Delegation of Authority. It cannot be altered or reassigned due to a manager’s unavailability.
